Documents required for Passport 2015

Applicants are required to present original documents along with one set of self-attested photocopies of the same at the Passport Seva Kendra (PSK) for processing of passport.

The following are the documents to be presented for a fresh passport.

a} Address Proof :- {Attach any one of the below}

1. Applicants Ration card (In case of ration card,you also need to attach any one amongst below)
2. Water/Telephone/Electricity bill
3. Statement of current running bank account
4. Voters Id/Election commission card
5. Gas connection bill

a.1} Minor

Spouse's/Mother/Fathers passport copy or present address proof document in the name of parents can be submitted. For minor applicants, documents can be attested by parents.

b} Date of Birth proof (Any One)

1. Birth certificate issued by Municipal authority or The district office registrar of Birth.
2. School leaving certificate or any authorised educational institute
3. Affidavit sworn before a Magistrate/Notary stating date/place of birth as per the specimen in ANNEXURE ‘A’ by illiterate or semi-illiterate applicants.
4. (In case of applicants born after 26/01/1989, only birth certificate will work.)

c}Citizenship Document if citizen is of India by Registration.

1. Date of birth Proof (mandatory)
2. Date of birth Proof of parents
3. Passport of parents or Voter ID Cards
4. Indian Citizenship Certificate for any other national
5. Marriage Certificate with spouse passport copy.

d} Education Qualification (Any one)

1. Last educational certificate
2. Admit card
3. Certificate of recognised educational institution

e} Public Sector/Government employees should submit their Identity Certificate in original (ANNEXURE B) along with Standard Affidavit Annexure I.

e} ECNR 

If the applicant is eligible for “ECNR” attach attested copy of supporting document

ECR stands for Emigration Check Required.

 ECR status will be printed in the passport of the applicants who fall in the ECR category.
